This form asks you to consent to the use of your image by the Interfaith (IFN) Network, of the City of Greater Dandenong. We use stories (words, photography, and film) in our communications and impact reporting activities. Your image may be used in our:

• Printed publications for promotional purposes
• Media releases
• Videos
• Social media channels
• Presentation materials
• IFN website
• Advertising

The image(s) will remain the property of the IFN and will only be used for increasing awareness about the IFN and the impact of our service and work. Your personal contact details will remain strictly confidential.

**In submitting my Membership Consent Release Form for the Use of Your Image, I consent to:

• The use of my image for the purposes listed above by the IFN.
• Granting IFN the right to reproduce, display, and disseminate my likeness in any traditional or electronic media format for the purposes stated.
• If I decide at any point that I no longer want my image to be used, I will contact the IFN Administration Officer via administration@interfaith.org.au.

*This Form is to be submitted with the IFN Membership Application Form and/or the IFN Membership Contact Information Form.

**Please note: if you do not Consent to the Use of Your Image by IFN, this does not exclude you from Membership and/or applying for Membership. Please ensure that you provide a response to your Consent or otherwise in your application for IFN Membership and/or the IFN Membership Contact Information Form.
